The AlzBetter coach is unlike anything that has come before it. Each coach has gone through extensive dementia training with a focus on behavior management.
Taking a holistic, patient-centered approach, the coach is uniquely qualified to advocate on behalf of the person living with dementia. offering support for family members while also coordinating additional services needed.    Â
Training & Education
Identifying most stressful behaviors and offering a customized, targeted solution to address each behavior
Helping to train the care providers and other members of the service provider staff
Offering care providers proprietary tools such as books, manuals and videos to allow for self-paced learning
Supporting Families
Providing coaching support and advocacy to family members as well as professional care partners
Regular outreach to family members to review the current status and address any new issues which may develop
A thorough dementia assessment is performed followed with the development of a customized plan of care
Community resources are offred to add additional support
Care Coordination and Advocating
Communication betwen healthcare professionals to make sure nothing falls between the cracks
Accompany you to doctors visits and other appointments
Scheduling appointments when necessary
Advocating for the patient or family when required
At Home or Facility
Helping people living with dementia to remain in their homes if that is their desire
Working with families as well as the facility staff when a person is located in Assisted Living, Nursing Home or other type of facility
