Add the name of the member you wish to send the AlzBetter App to. They will be sent a link to go to the appropriate app site where they can download the app.
Recipient Name(Required)
Name of recipient receiving the app link. This data will be HIPAA encrypted.
Member Name(Required)
Name of member receiving the support. This data will be HIPAA encrypted.
